Certificate: I hereby certify that the information provided above is true to the best of my knowledge. I am willing to remit Rs.1,000;- for ABS membership and the scheme admission fee @ Rs.1,000/- x "my age as on the date of application " to ABS bank account, if my application is accepted. I have read and understood the scheme details. I understand that the society will pay Rs.15,00,000/- to the nominee within 7 days of death of any member in the scheme and the amount will be equally contributed back to the society by the live members (excluding paid up members) of the society. Any member completing 25 years of membership without any dues will be considered as paid up member and need not pay further contributions. My scheme benefits will be applicable only after my admission fee is received in full by ABS. I also understand that my admission fee would be returned without interest to me in case of the scheme fails to achieve the required minimum membership of 300 numbers. I am not eligible for any benefit from the scheme if the scheme is withdrawn due to lack of membership.
